In establishing an advisory account with FAC D/B/A Cornerstone Management, Inc., it is necessary that you provide FAC D/B/A Cornerstone Management, Inc. with certain personal financial information necessary to ensure that your account is managed in a manner consistent with your investment objectives and personal financial goals. This information will generally be derived from the following sources:
- Information we receive from you on the Client Investment Questionnaire or other forms, and
- Information about your securities transactions with Cornerstone Management, Inc. or other advisers and broker/dealers, which may be providing financial services to you.
It is the policy of Cornerstone Management, Inc. that we do not disclose any non-public personal information about you to anyone, except as permitted by law. In circumstances where providing confidential information may be important we will provide you with the ability to "opt-out" of disclosing confidential information to a third-party. If requested to provide confidential information on your behalf to a third party (i.e. CPA, Mortgage Broker, etc.), we will require written authorization. In addition, we will require written authorization to utilize the name of your organization in any marketing information (i.e. Partial Client List).
If you decide to close your account(s) with FAC D/B/A Cornerstone Management, Inc. or become an inactive customer, FAC D/B/A Cornerstone Management, Inc. will adhere to the privacy policies and practices as described in this notice.
FAC D/B/A Cornerstone Management, Inc. restricts access to your personal and account information to those employees who need to know that information to provide products or services to you. FAC D/B/A Cornerstone Management, Inc. maintains physical, electronic, and procedural safeguards to guard your non-public personal information. This includes measures to protect your information in the course of its disposal.
